You obtain it from one of the regular or soul chest or the fire chest behind the shrine area after going down the stairs on the left.Just keep on opening chests and filling them again using kroniks's engine and you will eventually get it.That is how I got it.I also have ut Cetrion's head on her pike in the shrine area so it might have been a reward from that chest but I highly doubt it.
